The Windows Scan app creates a folder called Scans under the Pictures folder to save scanned documents and pictures.
To access scanned files, open This PC, click Pictures in the left pane, and then double-click the Scans folder to view all documents and pictures scanned using the Windows Scan app.

Windows Fax and Scan program saves all scanned documents and pictures in a folder called Scanned Documents under the Documents folder.
So, to access documents scanned using the Windows Fax and Scan app, you need to open the Documents folder first and then double-click on the Scanned Documents folder.
